Random thoughts:
There is no creativity in fear. Often the biggest threat to success in business isn't the competition--it's the fear that lives within your organization's own walls. Fear causes people to believe they need walls and barriers to protect themselves, even though the barriers make it harder for others to succeed. (Adapted from Tom Reiger, Gallup Inc.)

Often "simple rules" exist in an organization. They affect governance and management of the enterprise; they aren't written down anywhere, but they affect daily life. One example (in a real organization I know of) is this: "You can go off and be all 'important' as a leader or administrator, as long as you don't do anything that impinges on my practice."
